How to: Remove a Form header/footer from a form.
Solution:
In the Form Design view, select the 'View' menu, and select 'Form Header/Footer'.
NOTE: Form headers and footers CANNOT be removed separately, for example, just a form header or just a form footer cannot be removed. However, the undesired header or footer can be hidden.
1) If the Database window is not active, activate the Database window.
2) Click the 'Forms' tab in the Database window.

3) Click 'Design'. (The Form appears in Design view.)
4) Select the 'View' menu and select 'Form Header/Footer'.
NOTE: If the Form Header/Footer contains controls, a prompt appears to verify the removal of the header/footer and all the controls.
WARNING: Choosing the 'Yes' in the prompt permanently deletes the controls within the header and footer.
